Our specialist nursing and care professionals provide personalised support to children with serious or life-limiting conditions and their families when they need it the most.

Who we support
Demelza provides clinical care and wraparound support for children and young people with serious or life-limiting conditions, and their families.
We support children and families living in the Demelza catchment areas of Kent & Medway, East Sussex or South East London (specifically the boroughs of Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark).
Referrals can be made before birth up to the 16th birthday for non-urgent care and up to the 18th birthday for specialist nursing, such as end of life care.

Family services
Every family’s journey is unique, and at Demelza, we provide the right level of care and support when it’s needed most.
Demelza’s services are grouped within three Circles of Care. These Circles of Care are designed to ensure we offer a broad range of holistic services while allowing our specialist nursing and care professionals to provide personalised additional support to families when they need it the most.

Refer a family
If you're supporting a child or family who could benefit from our services, referrals can be submitted online or via a downloadable form.
Anyone can make a referral with parental consent, such as a health and social work professional, family liaison officer or a teacher. Parents or carers can also refer their child directly to Demelza.
