Transition services 14+

Turning 18 can bring changes as young people begin to move into adult social and healthcare services. At Demelza, we’re here to support families through this transition.

Emotional and practical support

Transition from Demelza’s services isn’t a single event, but a gradual process of supporting a young person, and their family, through their teenage years into adulthood.

We start planning from the age of 14 and continue to offer guidance and support up to a young person’s 25th birthday, helping to ensure continuity of care and a smoother journey into adulthood.

You’re not alone

Our teams have developed a pathway of support for the whole family, offering guidance and specialist services at key moments up until a young person reaches 25.

From how to navigate the system and make sure you get the support and funding you’re entitled to, to advice and activities for young people – we’ll help build your confidence and empower you to challenge the system and any decisions you feel aren’t the right fit for your family. 

Our support 

Our teams are specially trained to help make this important transition a positive one.

Practical advice and emotional support

We’re here to equip you with what you need to know, when you need to know it. Our transition pathway includes advice, support and direction on issues such as finances, deputyship, funding, educational programmes, personal budgets, supported living, further education and care plans. We’ll support you with decision making, providing guidance when facing difficulties.

Putting young people first

We offer age and developmentally appropriate advice and support for your child, keeping them at the centre of future decision making. We talk to them about their hopes and aspirations, such as further educational opportunities or assisted living applications. Through workshops, events and social activities with peers, we hope to build confidence, support networks, increase self-esteem and emotional resilience.

We advocate for families

We will advocate for you if you need support navigating different services or help with difficult situations. We’ll make sure you don’t feel alone as your child reaches 18.

Connecting with other families

If you’d like to connect with other families who are accessing transition services, build a support network, make friendships, ask questions and share experiences, we offer several forums and networks designed to bring together those on a similar journey.

Joined up working

We work with healthcare providers and adult hospice professionals on how to support young people with complex medical conditions and engage with a range of local organisations and agencies to give you choice and control over how and where to access adult services.

Educating others

We’re working hard to change attitudes towards what quality of life looks like, and to make transition to adulthood smoother and less stressful.
